An Advance Health Care Directive, sometimes called a Living Will, allows your Agent to make health care decisions for you if you cannot make those decisions yourself. Your Agent is the person you nominate to make health care related decisions for you. This document gives legal notice of your decisions regarding health care to your health care providers. Your Agent will be able to accept or refuse medical treatment, have access to your medical records, make decisions about donating your organs, authorizing an autopsy, and funeral arrangements, if any, as you direct.
